Community Health Workers (CHW)
The CHW service is managed and delivered by Otara Health Inc under contract to THO. CHWs receive referrals from THO’s GPs and Practice Nurses for a range of reasons including:
- To educate the patient to have a good understanding of their health, medications & specialist appointments
- To reach and engage with patients who have high needs and to provide support for them to access essential services
- To set goals with patients to support them to take positive action to improve their health
- To ensure that clients are successfully engaged with the relevant agencies that support positive outcomes, around housing, income, employment, social support, food etc.
- To support immunisations, screening and chronic care management appointments
CHWs play a critical role in the primary health care team.
